Observed 2 bags of shredded lettuce had internal temperature of 57F in standing cooler in sitting area (left of entrance to kitchen). Observed one bag of shredded carrots in same cooler also had an internal temperature of 57F.Corrective Actions: Ensure TCS food items are held at 41F or less. Manager discarded lettuce and carrots.

Observed standing cooler to the left of entrance to kitchen was holding between 50-65F. Temperature logs recorded temperature of cooler to be less than 40F only 10 minutes prior to inspection which indicates temperature logs were falsified.Corrective Actions: Ensure cooler is able to maintain a temperature of 41F or less. Do not use until repaired.
