Procedures for responding to vomiting and diarrheal events

Observed no procedures/bodily fluid kit on site during time of inspection.Corrective Actions: Ensure to obtain procedures/bodily fluid kit in case of vomiting/diarrheal events.

Food-contact surfaces: cleaned & sanitized

Observed sanitizer in 3-compartment sink and in sanitizing bucket testing at 0 ppm. Observed sanitizer dispenser not dispensing properly.Corrective Actions: Ensure to repair sanitizer dispenser and that sanitizer in 3-compartment sink and in sanitizing buckets is maintained at 200-400 ppm.

Proper hot holding temperatures

Observed chicken tenders, legs, thighs, and wings maintained under a heat lamp with internal temperatures ranging from 110-120°F.Corrective Actions: Ensure to hot hold all TCS items at 135 or above. Manager stated that all food items had just been placed under heat lamp twenty minutes prior and chose to discard them.

Contamination prevented during food preparation, storage and display

Observed raw chicken stored on the floor in the walk-in cooler closest to the back door.Corrective Actions: Ensure all food items are stored at least six inches above the ground at all times.

Wiping cloths; properly used and stored

Observed several wiping cloths stored out on countertops throughout kitchen.Corrective Actions: Ensure wiping cloths are stored in sanitizing buckets with sanitizer.

In-use utensils; properly stored

Observed scoops stored inside of bulk sugar containers with handles in direct contact with the food item.Corrective Actions: Ensure to properly store in-use utensils.

Physical facilities installed, maintained, and clean

Observed several damaged ceiling tiles throughout the facility.Corrective Actions: Ensure to repair/replace ceiling tiles.

Adequate ventilation and lighting; designated areas used

Observed light in walk-in cooler closest to 3-compartment sink not functioning.Corrective Actions: Ensure to repair/replace lightbulb in walk-in cooler.

Adequate ventilation and lighting; designated areas used

Observed condensation dripping from ceiling vent located directly outside of walk-in cooler.Corrective Actions: Ensure to have ventilation system repaired or address the source of condensation to prevent continued dripping.
