Observed several cases of sliced american cheese in reach in cooler below toppings section cold holding at 51F, items were in cooler for less than 2 hours.Corrective Actions: Ensure all TCS food items are cold holding at 41 F or below. Manager on site moved cheese containers to cooler with adequate refrigeration to cool down.

Proper cooling methods used; adequate equipment for temperature control

Observed the small reach in cooler below the toppings section holding an ambient temperature of 51F.Corrective Actions: Ensure to not store any TCS food items in cooler until serviced.

Plumbing installed; proper backflow devices

Observed an improper air gap of the pvc pipe connected to the high temperature dish machine. Corrective Actions: Ensure to raise pipe to 1 inch above the drain to avoid backflow.

Toilet facilities: properly constructed, supplied, cleaned

Observed no covered trash can in the women's bathroom Observed no employee hand washing signage in the men's bathroom.Corrective Actions: Ensure the women's bathroom has covered trash cans. Ensure to have employee hand washing signage in the men's bathroom.

Physical facilities installed, maintained, and clean

Observed ice build up on fans and on the floor of the walk in freezer.Corrective Actions: Ensure to remove ice buildup.
