  Observed perishable and non-perishable food items, received from an approved source, re-packaged on-site for consumer sale without proper labeling. Also, observed pre-packaged food items received from approved source not properly labeled with retail food establishment information.Corrective Actions: Must label all packaged foods on-site, with business name, address and phone number of retail food establishment. For food items re-packaged at the retail food establishment, must provide information for the establishment it was packaged from (example, Froth Coffee). For food items es received already packaged and sold as-is without re-packaging on-site, the retail food establishment it came from must provide labeling on the package.

  Observed plumbing to floor drain for three compartment sink not provided with adequate air-gapping.Corrective Actions: All plumbing leading to floor drains must be provided with adequate air-gapping (raised above floor drain), as to prevent potential wastewater back flow.
