  Observed raw chicken stored above carrots and water bottles in the walk-in cooler. Observed boxes of food stored on the floor. Manager rearranged cooler.Corrective Actions: Ensure all foods are stored at least 6" off of the floor. Ensure raw chicken is stored below all other food products and drinks.

  Proper cold holding temperatures

  Observed shrimp and egg rolls in the low boy cooler ranging from 44°F - 51°F. Foods were placed in the lowboy cooler at 8am.Corrective Actions: Ensure TCS foods remain at 41°F or below. Ensure egg rolls and shrimp are moved to the walk-in cooler at 11:30 for rapid cooling.

  Observed lowboy cooler is not keeping foods at 41°F or below. Manager states a repair service has been scheduled for today.Corrective Actions: Ensure cooler is capable of keeping foods at 41°F or below.

  Observed cracked floor tiles and missing grout in the back area. Observed exposed glass bulbs. Corrective Actions: Ensure to repair floors and cover exposed bulbs with a shatterproof covering.
