Observed cut tomatoes, butter, and other TCS foods in an open top cooler ranging from 54°F - 61°F. Foods were prepped between 10:30 - 11:00am today. Manager added ice to cooler and temperature began to rapidly decline. Several food items cooled to 41°F within 4 hours of prep. Manager disposed of lettuce and tomato that did not return to 41°F.Corrective Actions: Ensure TCS items remain at 41°F or below at all times. Ensure to dispose of food items that have been out of temp for 4 hours or longer.
