  Observed the salads and the pizzas were using time as a control and none of the timers were set.Corrective Actions: Ensure all time as a control for safety foods have a timer set as stated in the procedure.

  Observed several dead German roaches on the floor throughout the facility. Observed the pest control service was completed this morning.Corrective Actions: Ensure pest activity is cleaned daily prior to food preparation. Ensure all food is put in sealed container or refrigerators nightly and all food contact surfaces are sanitized daily prior to food preparation.

  Observed the hand sink by the 3 compartment sink was hanging off the wall.Corrective Actions: Ensure the hand sink by the 3 compartment sink is repaired within 24 hours.

  Observed several ceiling tiles out of place. Observed absorbent ceiling tiles above the hot water heater. Corrective Actions: Ensure the ceiling tiles are in place at all times. Ensure all the ceiling tiles are nonabsorbant.
