Obseved a few food workers not wearing hair restraints. Corrective Actions: Ensure all food workers are wearing hair restraints.

Certified Food Protection Manager / Food Handler Certification

Observed no certified food manager certificate on site at the time of inspection. The certified food manager did provide a screenshot of his valid food manager certificate. Corrective Actions: Ensure a valid certified food manager certificate is displayed in public view.

Proper eating, tasting, drinking or tobacco use

Observed an open container(for personal consumption) of a beverage next to a microwave inside the food prep area. Corrective Actions: Ensure perosnal use beverages are stored and consumed in a designated area in a manner to prevent the contamination of food, equipment, utensils, linens, and single-service/single-use articles.

Adequate hand washing facilities supplied and accessible

Obseved no soap inside the soap dispenser closest to the drive-thru window. Obseved no disposable towels inside the paper towel dispenser next to the three compartment sink. Observed ten trays stacked on top of a big steel container stored inside the hand wash sink next to the three compartment sink.Corrective Actions: Ensure all soap dispensers and paper towel dispensers are replenished. Ensure nothing is stored inside a designated hand wash sink. Hand washing sinks are solely for hand washing purposes only.

Food-contact surfaces: cleaned & sanitized

Obseved a black mold-like formation on the ice chute I did the ice maker. Corrective Actions: Ensure ice maker chute is throughly cleaned and sanitized at a frequency to prevent soiling.

Consumer advisory provided for raw / undercooked food

Observed no food allergen awareness poster displayed in the kitchen. Corrective Actions: Ensure a food allergen poster is displayed in the kitchen.

Plumbing installed; proper backflow devices

Obseved a drain pipe extended inside the floor drain. Corrective Actions: Ensure the pipe is fixed and positioned in a manner to provide an air gap of a minimum one inch for proper back flow prevention.
