Observed improper to no date marking on prepared TCS foods stored inside walk-in cooler and fridge units.Corrective Actions: Must provide clear date marking on prepared TCS foods, with date of preparation and disposal date not too exceed 7 days.

Observed use of plastic bowels without handles stored inside bulk food containers.Corrective Actions: Must only use approved scooping utensils with handles for bulk food containers, as to prevent contamination.

Single-use/single-serve articles; properly stored, used

Observed boxed single-service items stored on the floor surface in the storage "hallway" back room area.Corrective Actions: Must store all boxed items off the floor surface, for ease of cleanliness of the floors.

Hot and cold water available; adequate pressure

Observed only 90 degree F hot water at hand washing sink in men's restroom.Corrective Actions: Must provide a minimum of 100 degrees F hot water at all hand washing sinks, for proper hand washing procedures.

Physical facilities installed, maintained, and clean

Observed dust accumulation on walls and ceiling in front preparation area of kitchen, as well as floors not kept thoroughly cleaned around boxes of products in back storage "hallway" area.Corrective Actions: Must regularly and thoroughly clean walls, ceilings, and floors, as to maintain a sanitary environment.

Adequate ventilation and lighting; designated areas used

Observed personal cell phone stored on cutting board in kitchen area.Corrective Actions: Must store all personal belongings in a seperate designated area, as to prevent contamination of food contact surfaces.
