Observed soda dispensing nozzles for soda fountain at server area not kept thoroughly cleaned.Corrective Actions: Must regularly and thoroughly clean soda dispensing nozzles, as to prevent contamination of food product during use.

Contamination prevented during food preparation, storage and display

Observed multiple violations concerning with potential contamination during food storage, including a metal container of onions stored on shelving, leveled and next-to the mop sink (risk of chemical splash contamination), uncovered containers of cooked bacon stored on the bottom shelf in the walk-in cooler, and a dry cloth used to cover cut produce on the right-side cold holding unit.Corrective Actions: Must ensure all food, while stored, is kept covered as to prevent overhead and cross contamination, food is properly spaced and seperate from mop sink, as to prevent risk of chemical splash contamination, and to not use cloth to cover food products, as to prevent contamination due to the absorption of the material and it being not easily cleanable.

Plumbing installed; proper backflow devices

Observed inadequate air gapping to plumbing leading to the floor drain, underneath the three compartment sink.Corrective Actions: Must ensure there is an adequate air gapping to installed or introduced, from plumbing leading into floor drains, as to prevent potential wastewater backup.

Physical facilities installed, maintained, and clean

Observed multiple unapproved ceiling tiles installed in the food preparation kitchen area.Corrective Actions: Must ensure all ceiling tiles are smooth, non-porous and easily cleanable in food preparation and storage areas, for ease of cleanliness.
