  Observed prep line cooler not working as it is intended. Person in charge puts ice in side the food compartment to keep TCS items cool.Corrective Actions: Ensure to fix or replace prep line cooler to keep equipment, working adequately, and as intended.

  Observed outside clean out was overflowing causing a fly attraction and a health hazard and I the potential source of contamination.Corrective Actions: Person in charge corrected the outside sewer overflow on site. ensure to clean and sanitize any areas that may have been contaminated.
