  Food-contact surfaces: cleaned & sanitized

  Observed gnats on the soda nozzle at the soda machine.Corrective Actions: Ensure to wash rinse and sanitize soda nozzles before reusing .

  Toxic substances properly identified, stored, used

  Observed two cans of insecticide stored at the service counter, both approximately half empty when asked about their use and employee stated that insecticide is believed to be used for spraying gnats inside the establishment. Corrective Actions: The person in charge at voluntarily discarded the insecticide spray. Management was instructed that employees are not permitted to apply insecticide inside of food establishment. The use of pesticide must be conducted only by a licensed pest control operator and accordance with label instructions.

  Observed a significant amount of gnats on food items, near the mop sink and inside the soda nozzles at the soda machine.Corrective Actions: Ensure to have a professional pest control operator to address the issues with the gnats.

  Observed ice scoop,stored on top of the ice machine on a unclean surface.Corrective Actions: Emsure to store the ice scoop on a clean and sanitized are to,prevent any contamination.

  Observed dust debris build up in the walk in cooler on the soda racks.Corrective Actions: Ensure to increase cleaning frequency to maintain a clean surface to sight and touch.

  Observed the three compartment sink was full of cleaning equipment. Corrective Actions: Ensure to properly store the cleaning equipment in a designated area and not in the three compartment sink.

  Observed no paper towels at the hand sink in the bathroom.Corrective Actions: Ensure to supply the hand sink in the bathroom with soap and paper towels so employees and customer can properly wash and dry there hands.

  Observed ceiling tiles in the walk in cooler appear to be sagging and have a black substance on the edges. Observed Cobwebs in between the reaching coolers and on the food racks where they keep the 2 liters cokes.Corrective Actions: Ensre to fix or replace the sagging ceiling tiles in the walk in cooler. Ensure to dust and clean between the reach in coolers and the racks where the 2 liter cokes are kept.

  Observed dust debris build up,on the HAVAC cooling fan guards in the walk i cooler.Corrective Actions: Ensure to increase the cleaning freqof the walk in cooler fan guards to maintain a clean surface to sight and touch.
