  Observed several cleaning chemicals (one with no lid) stored below the hand wash sink in the public restroom. Observed room sprays, glue away and ant killer on s shelf above disposable plates and ziplock style bags- on the sales floorCorrective Actions: Seal all chemicals and keep in a designated area. Store food and food containers away and above all chemicals.

  1. Observed a gap between the back delivery/exit doors 2. Flooring in disrepair/floor tiles missing in several areas throughout the permitted space 3. Flooring unclean behind equipment 4. Walls around the back exit doors observed heavily damagedCorrective Actions: Seal all gaps in the facility to prevent pest entry. Repair floors and walls where necessary. All surfaces in a permitted space shall be smooth and easily cleanable.
