Observed no hand washing signage for the designated hand sinks.Corrective Actions: Ensure to have hand washing signage for all designated hand sink.

Observed black organic matter on the upper interior of the ice machine.Corrective Actions: Ensure to properly drain, clean, and sanitize the ice machine.

Observed chicken sitting in a small can thawing in still water.Corrective Actions: Ensure to thaw food products under running water with a temperature of 70F or below.

Observed boxes of french fries stored directly on the floor of the walk-in freezer. Corrective Actions: Ensure all food items are kept 6 inches odd the ground to avoid contamination.

Wiping cloths; properly used and stored

Observed little to no ppm in the sanitizer bucket for wiping cloths.Corrective Actions: Ensure to have sanitizer sanitizing between 200-400 ppm. Employee dumped out solution and refilled with adequate sanitizer range.

Observed no employees must wash hands signage in the restrooms.Corrective Actions: Ensure to have employees hand washing signage in the restrooms.

Physical facilities installed, maintained, and clean

Observed damaged and missing floor tiles throughout kitchen area.Observed several damaged/missing cove tiles below the hand sink next to the drive through. Observed damaged walls with paint chipping off behind the 3- compartment sink.Corrective Actions: Ensure to replace/repair.
