  Observed the cutting board above the frozen burger patties was pink and had deep grooves. Observed the ice machine had deep grooves and rust near the ice chute.Corrective Actions: Ensure the cutting board is repaired or replaced. Ensure the ice machine is repaired or replaced.

  Observed lettuce at 56*F in the prep cooler next to the grill. Observed bacon in the reach in refrigerator next to the 3 compartment sink at 48*F and hot dogs at 51*F and corn dogs at 44*F. The manager recently moved those items from the prep refrigerator that was not working. The manager will continue to monitor the items and ensure they are 41*F or lower or discarded in 3 hours.Corrective Actions: Ensure all temperature as a control for safety foods is kept at 41/ *F or lower at all times.

  Observed hot dogs in the sink thawing.Corrective Actions: Ensure to use an approved thawing method. 1) Frozen to cook 2) Frozen to refrigerate to cook 3) Frozen to submerged in water with water flowing over creating a waterfall effect.

  Observed the walk in freezer and refrigerators had particle growth on the electrical lines and fans. Observed water accumulation in the lights of the walk in refrigerator.Corrective Actions: Ensure the walk ins are cleaned regularly. Ensure the lights are repaired or replaced in the walk in refrigerator.

  Observed the sanitizer dispenser was damaged and not working. Observed the 3 compartment sink drain plugs did not work on any of the compartments.Corrective Actions: Ensure the sanitizer dispenser is repaired or replaced. Ensure the 3 compartment drains are repaired to ensure proper dishwashing.

  Observed the mop sink area needs cleaning. Observed food debris beneath the soda fountain canisters.Corrective Actions: Ensure the mop sink is cleaned. Ensure the floors under equipment is cleaned regularly.

Observed areas in the floor that are uneven and have stagnant water accumulation.Corrective Actions: Ensure the ceiling tiles are repaired/replaced and in place. Ensure the floors are smooth, durable, nonabsorbant, and easily cleanable.

  Observed a light cover missing above the 3 compartment sink. Observed what appears to be water accumulation in the lights of cover next to the walk in and 3 compartment sink. Observed a light cover damaged in the desk area.Corrective Actions: Ensure all lights are shatter proof or covered.
