Observed unused equipment and supplies in the back storage room.Corrective Actions: Ensure the floors are cleaned throughout the facility. Ensure the back storage area is cleaned.

Observed the restroom has aqoustical and absorbent ceiling tiles.Corrective Actions: Ensure to provide a covered trash can and toilet paper at all times in the restroom. Ensure the restroom has nonabsorbant, acoustical tiles, and easily cleanable ceiling tiles.

  Observed a large gap beneath the back door. Observed several damaged floor tiles throughout the facility. Observed water damaged ceiling tiles and out of place ceiling tiles. Observed some holes and cracks in the wall by the back door. Observed soiled mops by the restroom.Corrective Actions: Ensure the back door is fully sealed. Ensure the water damaged ceiling tiles are replaced and all ceiling tiles are in place. Ensure the floor tiles are replaced/repaired. Ensure all walls are fully sealed. Ensure mops are hung to dry and replaced when uncleanable.
