Common Name, Working Containers - Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies are not clearly and individually identified with the common name of the material.

Observed spray bottle with liquid in it on register counter. Person in charge provided name label at time of inspection.

Restriction - Poisonous or toxic materials that are not required for the operation and maintenance of a food establishment, such as for the cleaning and sanitizing of equipment and utensils and the control of insects and rodents, are allowed in the food establishment.

Observed can of insect killer under kitchen desk and 2 cans of insect killer under 3 compartment sink. Person in charge removed from establishment at time of inspection.

Openings to outside not protected against entry of pest as specified in the Food Code; and/or Protective barriers not provided for exterior walls/roofs

Observed light gap on right side of patio door.

Materials for floors/walls/ceilings not smooth, durable, & easily cleanable under normal use; and/or walls/ceilings are not light in color

Observed wall to the right of pizza oven behind racks to be in disrepair.

Physical facilities not maintained in good repair & cleaned as often as necessary to keep them clean

Observed stained walls and ceiling tiles throughout main kitchen. Observed ceiling tile holding ceiling vent in back unisex bathroom to be in disrepair.

Plumbing system not maintained in good repair

Observed leak under middle basin of 3 compartment sink. Person in charge fixed at time of inspection. ***REPEAT CRITICAL VIOLATION WILL BE ASSESSED IN 3-10 DAYS***
