Observed unpackaged frozen fish stored in deep freezer on back wall that was covered in ice. Observed dented can of tomato paste on shelf for sale.Corrective Actions: Ensure to discard fish in freezer that has been exposed to frost/ice. Ensure to discard dented can.

Proper date marking and disposition

Observed multiple food items such as fish leafy green vegetables that were missing labels and dates.Corrective Actions: Ensure all food items are properly labeled and have written expiration dates.

Food properly labeled; original container

Observed several food items for sale missing proper food manufacturers labels.Corrective Actions: Ensure all food items are properly labeled. Manager took food off shelf for sale.

Insects, rodents and animals not present

Observed mice droppings in bottom cabinet near entrance to mop room.Corrective Actions: Ensure to have pest control company treat for mice. Manager provided recent pest control visit and confirmed ongoing treatment of facility.

Warewashing facilities, installed, maintained, used, test strips

Observed tongs being used for fish in freezer but no 3-compartment sink was on site to properly clean.Corrective Actions: Ensure to not use utensils as there is no 3-compartment sink on site.

Non-food contact surfaces clean

Observed bottoms of 2 standing freezers and deep freezer had build up of residue and ice. Observed puddle of water and grime on shelf below ceiling AC conditioning unit.Corrective Actions: Ensure to maintain freezers clean and free of build up. Ensure to maintain shelf clean below AC unit.

Physical facilities installed, maintained, and clean

Observed dirt accumulation in corners of store, damaged floor throughout facility, leak from AC conditioning unit, and an unused display cooler in back of store.Corrective Actions: Ensure to maintain floors clean, repair leak from AC unit, and remove any unused equipment from facility.
